Electronic Health Record (EHR) — (a.k.a. electronic patient record  (EPR) or computerised patient record) defines a collection of electronic health information about individual patients or populations in digital format that can be shared between different medical establishments and across different health care settings, by being embedded in network-connected enterprise-wide information systems. Such a record may include a whole range of data in comprehensive or summary form, including demographics, medical history, medication and allergies, immunization status, laboratory test results, radiology images, vital signs, personal stats like age and weight, and billing information. This term is often used interchangeably with EMR (Electronic Medical Record), however there's a slight difference. The EMR refers to the legal patient record created in hospitals and ambulatory environments, it is the data source for the EHR. An EHR can be accessible by the user and healthcare providers.

The ISO/EN13606 norm has been developed to exchange EHRs between HCIS (Health Care Information Systems) in a semantic interoperable way and is able to represent all information included in EHRs. Its aim is to standardize the way EHR extracts are interchanged to guarantee interoperability.
